It will make "the saint" and "transmissionary" achievements impossible, and you don't have to save every survivor unless you want those. Just so you know for future plays, you have to take one picture of him taking a picture of you (worth 500 pp) and one of him posing with his fist in the air (700 pp). It won't change your ending at all but you won't be able to do his other two missions either
